 The area represented in Pars IV: Mediolanum (Segments III, IV) includes northern Italy, the heart of Cisalpine Gaul, portions of the Alps, the Adriatic coast, and part of Narbonne Gaul. Mediolanum (Milan) is a major road junction, visible in the central part of the segment.

The seas, rivers, and mountains are stylized. In this section, the Adriatic Sea appears as an horizontal blue stripe, with islands and schematic coastlines. The main cities and roads visible in this section are: Mediolanum (Milan) in the center-north, indicated as a major road junction; Bergomum (Bergamo), Brixia (Brescia), Verona, Cremona, Placentia (Piacenza), and Ticinum (Pavia). Regio (Reggio Emilia), Mutina (Modena), Parma, and Bononia (Bologna) are visible further south. To the east, the road continues towards Aquileia, Tergeste (Trieste), and the Adriatic coast. Various routes leading to the Alps are also illustrated (for example, to Mons Iovis, the Great St. Bernard Pass).

Alpine lakes and rivers such as Lacus Larius (Lake Como), Lacus Benacus (Lake Garda), and waterways such as the Po River are drawn in a stylized manner, and mountain ranges are represented with jagged lines. This section highlights the high urban density: Northern Italy was, in fact, one of the most urbanized and vibrant regions of the Empire. The commercial and military function of the road network is also highlighted, with Mediolanum as a crossroads between the Alpine region, the Po Valley, and the Italian peninsula.
